Forms
The Forms section contains electronic forms created by your specialist's organization. Forms can be used to gather additional information about you when you apply for programs or as a communication tool used by your specialist.
Once you complete filling out a form, you must enter your name to serve as an electronic signature before you can finalize. Once a form has been finalized, it cannot be edited, so be sure to check your work!
➺ If you do not see a tab called "Forms" this indicates that your organization does not allow you to access electronic forms via the Client Portal.
➺ You may see what forms are available to you by clicking the forms tab. This will list all the forms available to you.
