Forms
If you do not see a tab called "Forms" when you are logged into your account, the organization does not allow you to access electronic forms via the Client Portal.

See What Forms are Available to You
Forms will become available for you to fill out only when they are required when applying for a program.
Forms that you can fill out are available in the Forms section.
- Click the Forms tab from the navigation bar
- Available Forms are listed below
Fill Out a Form
Remember, Forms will become available for you to fill out only when they are required when applying for a program or have been started by a specialist.
- Click the Forms tab from the navigation bar
- Locate the form you want to fill out from the list below
- Click View
- Tip: forms that have been signed and finalized will read "Yes" in the "Finalized?" column and cannot be edited
- Fill in the form - be sure to review your answers before proceeding to the next step!
- Type your name in the Signature text field
- Click Finalize
- Form is completed
Finalize a Form
Remember, Forms will become available for you to fill out and finalize only when they are required when applying for a program.
- Click the Forms tab from the navigation bar
- Locate the form you want to finalize from the list below
- Click View
- Tip: forms that have not been signed and finalized will read "No" in the "Finalized?" column
- Ensure the form is filled in correctly - be sure to review your answers before proceeding to the next step!
- Type your name in the Signature text field
- Click Finalize
- Form is completed
View Previous Forms
Forms that you or your specialist have finalized are available for viewing via the Client Portal.
- Click the Forms tab from the navigation bar
- All Forms are listed below
- Locate the Finalized Form you want to view
- Tip: forms that have been signed and finalized will read "Yes" in the "Finalized?" column and cannot be edited
- Click View to see:
- Who created (begun) the form
- The Date it was created (begun)
- Who finalized it
- The date form was finalized
- The Form information itself
- When finished, click
All forms at the top of the page to return to all Forms
Print/Save Finalized Form
- Click the Forms tab from the navigation bar
- All Forms are listed below
- Locate the Finalized Form you want to Print or Save
- Tip: forms that have been signed and finalized will read "Yes" in the "Finalized?" column and cannot be edited
- Click View to see the form
- Click
Print Form in the top right corner - Computer prompt will open to print or save
- Print/Save the form as desired
- When finished, click
All forms at the top of the page to return to all Forms
